Overview Key Activities:
- Create a welcoming environment within our hubs, taking telephone calls and referrals.
- As part of a team, target known areas to assertively engage with rough sleepers/service users and respond to referrals where appropriate.
- Build positive relationships with rough sleepers, empowering them to achieve their goals.
- Carry necessary supplies such as food, paperwork, and other provisions.
- Attend outreach centres, community venues, and satellite services to engage and support service users/rough sleepers.
- Work closely with partner agencies and the community, including attending meetings where appropriate.
- Support rough sleepers/service users by signposting and advocating for them to access support, being a central person between them and other services.
- Ensure awareness of and follow the outreach risk assessment/risk management plans that are in place.
- Keep records of your contacts.
- Alongside staff, identify ways to remove barriers and find solutions to help engage rough sleepers/service users.
- Offer and provide overdose awareness information/naloxone kit.
- Provide confidential, easily accessible, non-judgmental advice around substance misuse, safer injecting, overdose, and what to do in an emergency.
- Work with staff to identify service users who have disengaged from services.
- Conduct home visits to engage service users.

- Help Change Grow Live fulfil its role in offering the best service to our clients.
- Perform your volunteering role to the best of your ability.
- Follow Change Grow Live’s procedures and standards, including health and safety and equal opportunities, in relation to our staff, volunteers, and clients.
- Maintain the confidentiality of Change Grow Live and of our clients; adhering to Change Grow Live’s policies and procedures.
- Meet the time commitments and standards which have been mutually agreed and give reasonable notice when this is not possible.
