We have an exciting opportunity for a Senior Worker/ Practitioner who has the skills and experience to assess and engage with our service users with complex needs. You will ensure that they access the right intervention at the right time, in a way that best meets their needs.

This fixed term role involves working with both drug and alcohol users who are experincing accommodation issues, often street homeless or being temporary housed. The role is challenging and you will support clients who have multiple issues. Working closely with Housing and other partners you will be flexible in your approach to engage with hard to reach individuals.

The role will involve outreach working and home visits, your main base will be at Thrift House in Hastings, however, you may be required to travel throughout East Sussex.You will attend weekly multi disciplinary meetings with the RSI team and be able to fully engage and be confident with your communication. You will be solution focussed and able to think outside of the box to ensure the best outcomes for the service users.

Full Time Hours: 37.5 per week

Full Time Salary: £32,002.35- £34,214.20 (pro rata for part time)

Contract: Fixed term contract until 31st March 2026

*Please note: Full-time hours at Change Grow Live are 37.5 hours per week. For part-time roles, the salary and payments will be pro rata based on contracted hours.

Responsibilities

About the role:

  • Identify, and engage with, people who have substance misuse problems with complex physical and / or mental health needs, including co-morbidity.
  • Support the development of service-based recovery communities.
  • Support the Team Leader to carry out case file audits and ensure findings encourage a culture of learning within the team.
  • Engage in regular consultation with service users to maintain the quality of the service, supporting staff by encouraging service user engagement.
  • Provide health education especially regarding harm minimisation, blood borne viruses and overdose prevention.
  • Ensure that all aspects of confidentiality are adhered to.

About you:

  • Excellent knowledge and understanding of drug and alcohol issues and experience of working within a related field.
  • Knowledge and experience of multi-agency working and establishing partnerships with other professions to achieve good outcomes for the service user group.
  • Ability to communicate complex information to individuals and groups in a way that is easily understood.
  • Written and verbal communication skills and a high degree of personal IT competency. The ability to accurately update and maintain records in a timely fashion and to work to deadlines for the submission of information.
  • Excellent team working and interpersonal skills, maintaining a highly cooperative approach to supporting colleagues in delivering service objectives.
