We have an exciting opportunity for a Senior Primary Care Veterans Worker who will increase awareness of mental health, substance misuse and physical health support and encourage the armed forces community to access support available to them.

Where: Hatfield

Part Time Hours: 22.5 per week

Full Time Salary Range: £32,002.35 – £34,214.20 (based on full time hours, pro rata for part time)

Contract Type:11 month Fixed Term Contract.

*Please note: Full-time hours at Change Grow Live are 37.5 hours per week. For part-time roles, the salary and payments will be pro rata based on contracted hours.

Responsibilities

About the role:

  • Support Veteran Friendly GP practices to develop action plans to improve access and outcomes for the armed forces community.
  • Support in recruiting more GP practices to become Veteran Friendly.
  • Support the standardisation of data reporting used by GPs to identify veterans.
  • Agree three local key service areas for development that will be the focus for transformation and improvement during 2025/26 and develop an action plan for each area.
  • Develop a Contact Directory of armed forces organisations and contacts.
  • Develop a Training Directory to record training undertaken by armed forces champions and leads.
