Change Grow Live is seeking an Assertive Outreach Recovery Coordinator at HMP Guys Marsh. The role is permanent, full-time (37.5 hours per week) with a focus on engaging hard-to-reach service users and guiding them into appropriate interventions.
You will support entry into the service, conduct assessments and recovery planning, and collaborate with stakeholders to reduce harm and promote meaningful social reintegration.
