Responsibilities

About the role:

  • Work alongside the Entry Into Service team with a focus on managing all prison leaver referrals for the relevant hub in the community
  • Complete and log accurately all relevant assessments and interventions such as triages, alcohol audits, SADQs, recovery assessments, criminal justice assessments, risk assessments, DBST and harm reduction amongst other interventions
  • Support with NMP medical appointments, nurse reviews and psychosocial support as and when required
  • Promote engagement with external professionals and agencies whilst maintaining a service user led focus around community involvement
  • Work and liaise with other agencies involved with the service user’s broader care plan including hospitals, GPs, community mental health teams, probation, police, prisons, housing and pharmacies
  • After completion of appropriate training or demonstration of relevant experience, carry out key harm reduction strategies such as offering Needle Exchanges and distribution of Naloxone
  • Adhere to safeguarding policies and complete outreach interventions such as home visits or joint appointments with associated professionals to clients, where required
  • Professional development through involvement with wider community hub activities by supporting open access responsibilities where possible

About you:

  • Experience in managing caseloads of people ideally within a CQC registered service or similar
  • An understanding of drug and alcohol issues and experience of working within a related field
  • An understanding of the criminal justice system and experience of working within a related field
  • A commitment to multi-agency working and establishing partnerships with other professions
  • Accomplished written and verbal communication skills and a high degree of personal IT competency
  • The ability to accurately update and maintain records in a timely fashion and to work to deadlines for the submission of information e.g. case management reports and high-risk referrals
  • Excellent time management skills and an ability to work on own initiative, prioritising accordingly
  • Excellent team working and interpersonal skills, maintaining a highly cooperative approach to supporting colleagues in delivering the wider service and specific team objectives
  • Good working knowledge of mental health interventions, services, and good practice
