You will work within the Clinical Substance Misuse team to develop and implement care plans and support pathways.
In this role you will hold a caseload for 11 keyworking, collaborate with prison staff and external agencies, and promote safeguarding and information governance. 37.5 hours, fixed-term 24 months.
