Your role will focus on identifying, engaging and supporting service users with drug and/or alcohol needs, delivering targeted interventions and robust release planning to reduce substance misuse, re‑offending and associated harm. You will work closely with prisons, probation services, and community‑based providers to ensure seamless transitions and timely access to appropriate support on release.
A fantastic opportunity has arisen to become a prison in‑reach worker on a fixed term basis, engaging service users who are engaged in a cycle of re‑offending. This role involves weekly travel to our feeder prison, HMP Hewell, therefore access to a car and a valid clean driving licence is essential for this role.

Hours: Full time, 37.5 hours per week

Responsibilities
- Act as a key liaison between prison in‑reach teams and community drug/alcohol services to support continuity of care, shared case management, and coordinated risk reduction planning.
- Carry out prison‑based screenings, initial assessments, and risk assessments, developing recovery plans in partnership with service users prior to release.
- Provide harm reduction education, overdose prevention, and relapse management planning with service users in custody and following release into the community.
- Deliver and promote Naloxone provision pre‑ and post‑release, and train service users and peers in its use where appropriate.
- Offer “through‑the‑gate” support including gate pickups where appropriate and facilitate access to community recovery services, housing, and health appointments upon release.
- Conduct home visits, hostel visits, and community outreach as needed to maintain engagement, particularly with high‑risk individuals and those at risk of disengagement.
- Work collaboratively with police, probation, mental health, housing, and social care partners to address the complex needs of high‑risk service users and promote recovery and rehabilitation.
