Are you experienced, skilled, and passionate about making a meaningful difference? We\'re seeking a dedicated Primary Care Link Worker to engage and assess our service users, ensuring they receive timely, personalised interventions that cater to their unique needs. In this role, you would be responsible for working alongside primary care including GPs and Hospitals, as part of the outreach substance misuse team. Developing and building successful working relationships within primacy care settings to support and identify the unmet need in the Knowsley areas.

Our ethos is centred around the principles of harm reduction, providing non-judgmental, fact-based information and advice surrounding drug and alcohol use and the associated risks.

Hours: Full-time (37.5 hours per week)

Salary: £27,861.26 - £32,002.35 dependent on experience (based on full time working hours, pro rata for part time hours)

Full-time hours at Change Grow Live are 37.5 hours per week. For part-time roles, the salary and payments will be pro rata based on contracted hours.

Responsibilities

About the Role:

  • Provide evidence based harm reduction services, including needle exchange;
  • Assist and carry out appropriate screening processes including urine and saliva tests.
  • Provide support for home-based treatments and provide education and support to patients, their families and significant others.
  • Work with partnership organisations to develop and sustain effective comprehensive treatment pathways and broaden the client’s recovery plan accordingly.
  • Attend GP multi-disciplinary meetings and contribute to patient specific care plans to improve health and risk of hospital attendance.
  • Ensure that all safeguarding issues, including child protection and protection of vulnerable adults, are dealt with in accordance to CGL policies and procedures, and local and national statutory requirements.
  • Present a professional image and actively promote the service at all times to service users, other stakeholders, partnership organisations and the general public.
  • To assess clients needs as required using evidenced based tools and making onward referrals to specialist agencies, where necessary.
  • What we are looking for:

  • Experience of Drug and Alcohol Treatment services or other similar background.
  • Extensive previous experience in Alcohol Liaison Work.
  • Excel in both verbal and written communication, with proficient IT skills.
  • Demonstrate proactive qualities, working independently with excellent time management and prioritisation skills.
  • Hold a strong understanding of mental health interventions, services, and best practices.
  • Thrive as a supportive team player with exceptional interpersonal skills, capable of collaborating with a diverse range of professionals, agencies, and internal/external stakeholders.
  • Exhibit the ability to navigate change effectively, always prioritising the needs of service users.
  • Recognise the significance of information governance processes and commit to adhering to all necessary safeguards.
  • Pursue continuous learning opportunities to enhance your professional knowledge and skills, while also contributing to the growth and development of your colleagues.
