We are looking for an experienced Think Family Worker to assess and engage with our service users, ensuring they receive the right intervention at the right time. The role supports the Norwich team in identifying users with complex safeguarding and family involvement, including pregnant individuals, and in providing tailored care plans. Home visits are a requirement of this role, so a driving licence is essential.
About the role
- Work with service users and their families to support social (re)integration, enabling meaningful and purposeful lives and promoting recovery, resilience, peer support and self-determination.
- Manage a caseload of individuals with complex safeguarding needs, including pregnant users and those with children in social care.
- Ensure that safeguarding practice is always followed, working in partnership with statutory agencies.
- Provide assessment, psychosocial interventions, integrated care planning and after-care support, including transfer of care.
- Advocate for service users to support engagement with partnership services.
- Develop strong relationships with partner agencies, ensuring robust referral protocols and care pathways for the effective identification and referral of those requiring ‘hidden harm’ interventions.
- Represent the service at partnership meetings such as MARAC meetings, child in need and child protection meetings.
About you
- Strong understanding of substance misuse issues and experience working in a similar field.
- Experience working with service users who have multiple risk factors such as domestic violence, anti-social behaviour, unemployment, unsuitable housing, pregnancy-related harm and hard reduction experience.
- Experience working in partnership with key stakeholders including Children and Adult social services and safeguarding boards.
- Proactive, self‑initiated, with excellent time‑management and prioritising skills.
- Good working knowledge of mental health interventions, services and best practice.
- Supportive team player with strong interpersonal skills and ability to work with a wide range of professionals, agencies and stakeholders.
- Ability to manage change successfully, prioritising the needs of service users.
- Commitment to information governance processes and adherence to all necessary safeguards.
- Experience in comprehensive assessments, care planning, case management and reviews.
- Sound knowledge of Child Protection procedures.
Essential: Driving licence.
