We're seeking a dedicated Hospital Liaison Healthcare Assistant to join our compassionate and diverse team in Kings Lynn team- a group of professionals committed to supporting individuals at the Queens Elizabeth Hospital Kings Lynn with compassion.

The post holder will work across the hospital site (e.g. A&E and admissions wards) to provide rapid access to assessment, brief interventions/harm minimisation and support discharge planning/onward referrals for people attending hospital with substance misuse related issues.

Where: Kings Lynn (Queen Elizabeth Hospital), Norfolk

Hours: full time, 37.5 hours per week

Full-time hours at Change Grow Live are 37.5 hours per week. For part-time roles, the salary and payments will be pro rata based on contracted hours.

Responsibilities

About the role:

  • Working closely with your Nursing team to develop pathways to care
  • Support service users in opiate and alcohol interventions within a hospital setting.
  • You will support on harm reduction interventions along with physical and mental health assessments.
  • Keeping the people who use our services at the heart of everything you do

About you:

  • Good interpersonal skills and a willingness to work flexibly as part of a team
  • Experience of working within substance misuse or a social care setting would be an advantage but not essential
  • A clear understanding of the need for and ability to deliver quality services
  • Good organisational skills
  • Confidence to work on own initiative as well as part of the team
  • Communicate confidently and effectively, verbally and in writing
  • Respond flexibly to the demands of the post
  • It is desirable that you have a full UK driving license as travel within Norfolk will be required for this role.
