We are now seeking a caring and experienced Recovery Coordinator to be part of a dynamic Severe Multiple Disadvantage Team within the Mental Health Pathways. You will be based within the County South Crisis Resolution Team supporting individuals into treatment as well as providing harm reduction advice and supporting the CRHT around substance and alcohol use.

Where: Highbury Hospital, Nottinghamshire

Hours: Full time, 37.5 hours per week

Contract Type: Fixed Term until 31st of March 2027

Responsibilities

About the Role:

  • Support joint working between Mental Health and Substance Misuse Services, including CMHTs and Psychological Therapies services.
  • Provide case management for individuals with both substance misuse and mental health needs.
  • Monitor mental health in collaboration with Mental Health professionals to develop joint care plans.
  • Create recovery-focused plans, support multi-agency involvement, and make referrals where needed.
  • Work flexibly across operational, satellite, and community sites.
  • Deliver group interventions in inpatient and community settings.
  • Maintain effective working relationships with relevant agencies and professionals.
  • Provide intensive engagement and social reintegration support in hospital and community settings.
  • Deliver brief interventions, advice, and referrals into local substance misuse services.
  • Attend joint MDT meetings and provide advice, support, and training to hospital staff on alcohol awareness, screening, assessments, and referral pathways.
  • Promote harm reduction and overdose prevention through increased Naloxone provision on discharge where appropriate.

About You:

  • Possess knowledge and experience in delivering interventions for service users affected by substance use.
  • Demonstrate proactive qualities, working independently with excellent time management and prioritisation skills.
  • Hold a strong understanding of mental health interventions, services, and best practices.
  • Thrive as a supportive team player with exceptional interpersonal skills, capable of collaborating with a diverse range of professionals, agencies, and internal/external stakeholders.
  • Relevant experience supporting vulnerable individuals, including those affected by substance misuse and mental health issues.
  • Strong working knowledge of substance misuse and mental health.
  • Experience supporting individuals with challenging behaviour, using effective de-escalation and conflict resolution techniques.
  • Competence in using case management systems and maintaining accurate, timely records and data.
  • Understanding of mental health interventions and recognised best practice.
