We have an exciting opportunity for a Housing Support Recovery Worker who has the skills and experience to assess and engage with our service users, ensuring that they access the right intervention at the right time, in a way that best meets their needs. The role will also involve engaging services users and supporting them to navigate treatment, working in partnership with Housing Officers to assist with addressing substance misuse issues by promoting engagement and whilst working with Housing Officers to identify those clients that exist within our services who need support with tenancy sustainment.
As directed by the Team Leader, the post-holder will be an integral member of a multi-disciplinary team promoting STAR and engaging service users and delivering psycho-social interventions, supporting clients to navigate clinical treatment pathways. The post holder will co-locate between CGL substance misuse settings and housing offices as needed. The post-holder will be part of a team that provides screening, comprehensive assessments, and care planning for new service users in outreach, delivering harm minimisation training and interventions as well as coordinating treatment for clients identified as having a housing need.

ResponsibilitiesAbout the role:
- Establish effective communication and referral pathways for individuals who are at risk of homelessness and in contact with other health and care agencies.
- Provide wrap around support in emergency/temporary/supported accommodations across the area.
- Prioritise caseload depending on crisis intervention and severity of needs.
- Attend relevant multi-disciplinary team meetings supporting links to health and social care services, peer support networks, community assets and mutual aid.
- Work closely with the local authority, Health.
