Change Grow Live is a leading substance misuse charity where our core values ‘Be bold, be open, be compassionate’ are guiding principles of our team’s efforts. We’re seeking a dedicated Hospital Liaison Healthcare Assistant to join our diverse team in Kings Lynn at the Queen Elizabeth Hospital.

The post holder will work across the hospital site (e.g. A&E and admissions wards) to provide rapid access to assessment, brief interventions/harm minimisation and support discharge planning/onward referrals for people attending the hospital with substance misuse related issues.

Location: Kings Lynn (Queen Elizabeth Hospital), Norfolk.

Hours: full time, 37.5 hours per week. Part-time roles will be pro rata.

About the role:

  • Work closely with the nursing team to develop pathways to care.
  • Support service users in opiate and alcohol interventions within a hospital setting.
  • Deliver harm reduction interventions and conduct physical and mental health assessments.
  • Keep people who use our services at the heart of everything you do.

About you:

  • Good interpersonal skills and a willingness to work flexibly as part of a team.
  • Experience working within substance misuse or social care is an advantage but not essential.
  • A clear understanding of the need for and ability to deliver quality services.
  • Good organisational skills.
  • Confidence to work on own initiative and as part of a team.
  • Excellent verbal and written communication skills.
  • Responsive and flexible to the demands of the post.
  • Full UK driving licence is desirable as travel within Norfolk is required.
