The role covers Eastbourne, with hubs in Uckfield and Crowborough, and involves coordinating clinical, psychosocial and community support to promote recovery.
You’ll conduct assessments, develop recovery plans, and work with a multidisciplinary team to reduce harm, manage risk and monitor progress.
