Change Grow Live is offering a permanent, full-time role in West Sussex DAWN Service as a Drug and Alcohol Family Practitioner. You will support families to access treatment, coordinate access within a multidisciplinary team, and deliver evidence-based interventions, including home visits.
A full UK driving licence and access to your own car are required. Based in West Sussex with co-location to West Sussex County Council; 37 hours per week with pro rata adjustments for part-time roles.
