We're looking for a Homelessness Outreach Worker to join our North Northamptonshire team, based in Wellingborough, supporting people experiencing rough sleeping, homelessness, substance misuse and multiple disadvantage. Why this role matters

Homelessness is rarely just about housing. Many of the people we support are experiencing a combination of substance misuse, poor physical and mental health, social isolation, trauma, poverty and exclusion. As a Homelessness Outreach Worker, you'll work directly with people who are rough sleeping or at risk of rough sleeping, helping them access support, reduce harm and move towards safer, more settled lives. You'll build relationships with people who may have disengaged from services, advocate on their behalf and help them navigate pathways into accommodation, treatment, healthcare and wider community support. Working alongside local partners, housing providers, health services and community organisations, you'll help create coordinated responses that focus on people's strengths, aspirations and long‑term outcomes. Most importantly, you'll ensure the people we support are heard, respected and empowered to make sustainable changes in their lives.

Who we're looking for

We're looking for someone who combines compassion with determination. You'll have experience of working with people who face complex challenges and understand how to build trust with individuals who may have experienced trauma, exclusion or repeated setbacks. You may have experience in:

  • Homelessness services
  • Substance misuse services
  • Mental health services
  • Criminal justice settings
  • Housing or community‑based support services

You'll also bring:

The Details

Role: Homelessness Outreach Worker
Location: Wellingborough, North Northamptonshire
Contract: Permanent
Hours: Full Time, 37.5 hours per week

Please note: Full‑time hours at Change Grow Live are 37.5 hours per week. For part‑time roles, salary and payments will be pro rata based on contracted hours.

Responsibilities

What you'll be doing: You'll spend much of your time out in communities, building relationships and connecting people with the support they need. Your role will include:

  • Identifying and engaging people who are rough sleeping or at risk of homelessness
  • Delivering assertive outreach and supporting people who may be reluctant to engage with services
  • Supporting people to access accommodation, healthcare, treatment and recovery services
  • Working closely with drug and alcohol services to reduce harm and improve wellbeing
  • Building strong partnerships with housing providers, local authorities and health services
  • Supporting individuals to identify strengths, build resilience and achieve personal goals
  • Advocating for people experiencing multiple disadvantage and ensuring their voice is heard
  • Maintaining accurate records and contributing to service performance and outcomes
