Are you passionate about working with people who have challenges with drugs and or alcohol and making a real difference in the lives of people facing some of society's most complex challenges? If so, we'd love to hear from you.
As part of the OASIS Team at Horizon Blackpool, you will support adults experiencing multiple disadvantage, including homelessness, substance misuse, mental health challenges, domestic abuse, and involvement with the criminal justice system.
Working within a dynamic multi-agency team, you will use trauma-informed and person-centred approaches to help individuals stabilise their circumstances, improve their wellbeing, and achieve positive, sustainable outcomes.
Where: Blackpool
Hours: Full time, 37.5 hours per week
Contract: Permanent
Full-time hours at Change Grow Live are 37.5 hours per week. For part-time roles, the salary and payments will be pro rata based on contracted hours.
Responsibilities
About the Role
This is an opportunity to be part of the innovative team and wider support network, helping people who often face significant barriers to accessing services.
You will work closely with our partners, Renaissance and Empowerment supporting people to build trust, inspire hope, and engage individuals who may have previously struggled to access support.
Together, you will:
Build positive and empowering relationships with people experiencing multiple disadvantage.
Connect individuals to the services and support they need.
Advocate on behalf of clients to ensure their voices are heard.
Deliver coordinated, holistic, and integrated support tailored to individual needs.
Collaborate effectively with partners across health, housing, social care, criminal justice, and community services.
