Overview
An exciting opportunity has arisen to join Change Grow Live substance misuse service in Warwickshire as an Aftercare Navigator (Recovery Champion).
CGL Warwickshire is a fully integrated recovery focused substance misuse service working to provide drug and alcohol treatment. The service delivers a range of services – from open access, harm reduction, medically assisted recovery and specific recovery interventions, group work and Criminal Justice interventions.
The service comprises of a range of interventions including key work and clinical co-ordination, psychosocial interventions, group work, family work and complex needs.
The role will involve working with service users to support them in building and utlilising recovery capital to move into long term recovery, supporting and delivering PODs or one to one interventions. The ability to work within a multi-disciplinary team is also essential.
You will be a positive role model ideally in recovery or have lived experience, supporting service users and promoting visible recovery by initiating and maintain links with Warwickshire\’s recovery community and mutual aid. In addition, you will promote service user involvement across the service to ensure we are providing the best service we can.
In addition to a sound understanding and knowledge of evidence based drug and alcohol related issues and interventions; the successful candidate will need to demonstrate a real passion and commitment to delivering services within a recovery and outcome focused environment.

Part-Time Hours: 18.75 per week
Full-Time Salary Range: ÂŁ25,077.00 (pro rata for part-time hours)
*Please note: Full-time hours at Change Grow Live are 37.5 hours per week. For part-time roles, the salary and payments will be pro rata based on contracted hours.
Responsibilities
About the role:
- Improve the outcomes for people exiting structured treatment by supporting service users to access the appropriate intervention, treatment and support at the earliest opportunity.
- Support the service user to connect with activities, support to sustain recovery and reach their goals set out in their aftercare plan e.g. training, employment support.
- Undertake clear, recovery focussed, trauma informed support and management of risk.
- Provide service users with information on options available them within the service and in their communities.
- Maintain up to date, timely records on CRIiS (Data Management System).
- Provide service users with advice and information to keep themselves safe.
- Reduce barriers to engagement and be flexible in your approach to the accessing support. This may involve addressing language barriers, conducting outreach or virtual sessions.
About you:
Experience of conducting and developing personalized Service User / Aftercare plans.
- Experience of delivering or supporting to deliver interventions.
- Experience of inputting onto case management systems.
- Knowledge of Warwickshire
