About the role

Are you passionate about promoting independence and delivering high‑quality person‑centred care? We’re looking for a motivated and experienced Team Leader to join our Targeted Care & Enablement Service – a short‑term, responsive service that plays a critical role in supporting people to regain skills, remain independent at home, and avoid unnecessary hospital admissions.

This is a dynamic and rewarding leadership role where you’ll lead a patch‑based team delivering enablement, bridging, and fast‑response care services across Ceredigion.

We’re seeking someone with strong partnership, communication, and analytical skills, and experience in social care commissioning or service co‑ordination. In return, you’ll join a supportive, forward‑thinking council that values innovation, flexibility and community impact.
