We are seeking an organised, compassionate and proactive Care Coordinator to join our domiciliary care team. You will play a vital role in ensuring high-quality care is delivered to our service users by effectively scheduling care visits, supporting care staff, and maintaining strong communication with clients and families.

Key Responsibilities

  • Coordinate and schedule care visits using care management systems
  • Match carers to service users based on skills and preferences
  • Handle rota planning, including sickness and emergency cover
  • Communicate with clients, families and care staff professionally
  • Ensure care plans are accurate and up to date
  • Support onboarding and training of care staff
  • Monitor compliance with CQC standards and company policies
  • Handle enquiries and concerns in a timely, empathetic manner
  • Maintain accurate records and reports
  • Take part in On Call duties

About You

  • Previous experience in domiciliary care coordination (preferred)
  • Strong organisational and time management skills
  • Excellent communication and people skills
  • Confident using scheduling and care management software
  • Calm under pressure and able to manage competing priorities
  • Knowledge of CQC regulations and care standards
  • A compassionate and professional approach
  • Must be a driver with a vehicle
