We are seeking a skilled Physiotherapist (PO3) to join the Access and Independence Service, supporting Haringey residents to achieve greater independence, wellbeing, and integration within their communities. You will provide high-quality assessments, deliver evidence-based interventions, and play an essential role in promoting the Adult Social Care vision across the borough. This position is ideal for someone who is values-driven, collaborative, and confident working in fast-paced, multidisciplinary environments.

Key Responsibilities

  • Assessment & Case Management
    • Carry out holistic assessments of residents with complex care and therapy needs, including mobility, balance, transfers, and functional ability.
    • Support service users to develop client-centred goals that promote independence and wellbeing.
    • Provide falls assessments, urgent home assessments, and assessments to support hospital discharge.
    • Assess and prescribe suitable mobility aids and equipment to support independence in daily living.
    • Identify long-term care needs and present professional recommendations to the Care Authorisation Panel.
    • Assess capacity and ability to make informed decisions in line with the Care Act and Mental Capacity Act.
    • Complete carers' assessments and identify support required to prevent carer breakdown.
    • Assess suitability for Continuing Health Care (CHC) referrals.
    • Support triage duties and contribute to identifying the correct pathways into Adult Social Care.
  • Therapy Intervention
    • Deliver evidence-based physiotherapy treatment programmes.
    • Teach service users and carers alternative methods to maximise independence and safety.
    • Recommend adaptations, specialist equipment, and cost-effective interventions in line with eligibility criteria.
    • Manage a caseload independently using strong prioritisation and time-management skills.
    • Conduct regular goal reviews and maintain progress updates.
  • Collaboration & Liaison
    • Work effectively with multidisciplinary teams including NHS Trusts, Mental Health Services, GPs, Occupational Therapists, and voluntary organisations.
    • Promote the physiotherapy service to external partners and provide professional guidance where required.
    • Maintain strong relationships across Adult Social Care and partner agencies.
  • Administration & Record Keeping
    • Maintain accurate, up-to-date case records using Mosaic and other council systems.
    • Produce high-quality written reports and maintain statutory documentation.
    • Ensure compliance with the Data Protection Act, GDPR, and Caldicott guidelines.
  • Training, Supervision & Professional Development
    • Provide professional supervision to Occupational Therapy Assistants and therapy students.
    • Contribute to training, induction programmes, and team development activities.
    • Act as a skills resource for colleagues across the service.
    • Participate in performance appraisals, CPD, and in-service training.
  • Other Duties
    • Recommend improvements to service delivery.
    • Ensure compliance with Health & Safety legislation.
    • Keep up to date with relevant legislation, policies, and sector developments.

Requirements

Ideal Candidate Profile

  • Essential Skills & Qualifications
    • Diploma or BSc in Physiotherapy.
    • HCPC / State Registration.
    • Experience delivering therapy assessments and interventions in clinical settings.
    • Ability to manage risk and identify safeguarding concerns.
    • Strong communication skills with service users, carers, and professionals.
    • Ability to produce high-quality reports and written assessments.
    • Experience working in a multidisciplinary team.
    • Knowledge of statutory social care legislation and current issues.
    • Ability to provide leadership and support to junior staff.
    • Commitment to equalities, inclusion, and high-quality service delivery.
  • Desirable
    • Post-qualification experience in a local authority, community, or acute setting.
    • Evidence of CPD and post-qualification training.
    • Experience managing performance and staff development.
    • Experience handling difficult conversations with service users or families.
    • Awareness of budget constraints and cost-effective service delivery.
    • Knowledge of HR processes and performance management.
