Main duties of the job

We are looking for a caring, enthusiastic and motivated Practice Nurse to join our friendly multidisciplinary team. This is an exciting opportunity for an experienced nurse, to play a key role in delivering high quality clinical services to our patients.

The successful candidate will work closely with GPs, healthcare assistants and the wider practice team to provide evidence-based nursing care and support the management of both acute and long term health conditions.

Job responsibilities

The following are thecore responsibilities of the Practice Nurse. There may be, on occasion, arequirement to carry out other tasks. This will be dependent upon factors suchas workload and staffing levels:

a. Tobe instrumental in the support of the Infection Prevention Control (IPC) leadand to work to the IPC requirements by providing audits, training, support andevidence of compliance to the practice manager

b.Tosupport in the publication of the IPC annual statement in conjunction with thepractice manager

c.Todevelop, implement and embed an effective care management programme for thefrail and housebound patients registered at the practice

d.Toassess the needs of patients ensuring the provision of healthcare isappropriate, incorporating evidence-based practice

e.Todevelop, implement and embed health promotion and wellbeing programmes

f.Toimplement and evaluate individual treatment plans for chronic disease patientsthat promote health and wellbeing

g.Toidentify, manage and support patients at risk of developing long-termconditions, preventing adverse effects to patients health

h.Asrequired, to provide routine nursing care to patients in accordance withclinical based evidence, NICE and the NSF

i.Asrequired, to diagnose and manage acute and chronic conditions

j.Providewound care (ulcer/Doppler, etc.) to patients

k.Toreview patient medications to enhance compliance

l.Toprovide guidance to patients in the use of prescribed and over-the-countermedications regarding side effects and dosages

m.Toco-ordinate the provision of travel medicine services

n.Torequest pathology services and process pathology results as required

o.Tooversee the provision of chronic disease clinics, delivering patient care asnecessary, referring patients to secondary/specialist care as required

p.Tomaintain accurate clinical records in conjunction with extant legislation

q.Toensure SNOMED CT codes are used effectively by all members of the nursing team

r.Toensure the appropriate maintenance and use of chronic disease registers

s.Todevelop, implement, embed and participate in well woman clinics

t.Todevelop, implement, embed and participate in well man clinics

u.Toensure compliance with the practice chaperone policy

v.Toassist GPs with minor surgery when required

w.Toprioritise health issues and intervene appropriately

x.Tosupport the team in dealing with clinical emergencies

y.Torecognise, assess and refer patients presenting with mental health needs

z.Toimplement vaccination programmes for adults and children

aa. To support patients inthe use of their prescribed medicines or over-the-counter medicines (within ownscope of practice)

bb. To liaise with externalservices/agencies to ensure the patient is supported appropriately (vulnerablepatients etc.)

cc. To delegate clinicalresponsibilities appropriately (ensuring safe practice and the task is withinthe scope of practice of the individual)

dd. To support the clinicalteam with all safeguarding matters, in accordance with local and nationalpolicies

ee. To understand practiceand local policies for substance abuse and addictive behaviour, referringpatients appropriately

ff.Todeliver opportunistic health promotion where appropriate

gg. To conduct patient homevisits as required

In addition to theprimary key responsibilities, the Practice Nurse may be requested to:

a.Developand review audit protocols and process for the practice, effectively utilisingthe audit cycle

b.Develop,review and implement nursing protocols in conjunction with the partners

c.Supportall members of the nursing team, providing guidance when necessary, conductingappraisals and training needs analyses as required

d.Participatein local initiatives to enhance service delivery and patient care

e.Supportand participate in shared learning within the practice in order to improvepatient care

f.Continuallyreview clinical practices, responding to national policies and initiativeswhere appropriate

g.Participatein the review of significant and near-miss events applying a structuredapproach, i.e., root cause analysis (RCA)

h.Coordinatethe nursing team rota, ensuring sufficient staff are available to meet patientneeds

i.Leadthe planning and implementation of changes within the nursing team, enhancingthe provision of services to patients

j.Supportthe practice team with the reviewing and implementation of practice policiesand protocols, ensuring conformance to extant legislation

Person Specification

Experience

• Experience of working in a primary care environment
• Experience of working autonomously
• Experience of working as a practice nurse
• Experience of infection prevention and control measures
• Clinical knowledge in the following:
• Wound care/removal of sutures and staples
• ECGs
• Venepuncture
• New patient medicals
• Immunisations (routine, childhood and travel)
• Requesting pathology tests and processing the results, advising patients accordingly
• Chronic disease management awareness for:
• Diabetes
• CHD
• Asthma
• Understanding the importance of evidence-based practice
• Understand the requirement for PGDs and associated policy
• Ability to record accurate clinical notes
• Ability to work within own scope of practice and understanding when to refer to GPs
• Chaperone procedure
• Knowledge of health promotion strategies
• Excellent communication skills (written and oral)

Qualifications
