Job Overview

To provide urgent high-quality, short-term programmes of care for vulnerable adults in their own home with acute health needs, and to facilitate timely discharge from urgent and acute care settings. Work within professional standards and clinical guidelines, promote best practice and undertake all aspects of clinical duties as an autonomous practitioner including assessing and treating own caseload of patients/clients and maintaining professional documentation.

Main duties of the job include:

• Undertaking specialised multi-professional assessments, developing and implementing treatment plans for clients referred to the Unplanned Care services.
• Participating in triaging referrals and allocation of visits appropriately.
• Communicating relevant medical information, assessment details, advice and recommendations across agencies and with clients in compliance with trust guidelines for sharing of information and informed consent.
• Using advanced clinical reasoning, analysis, assessment and reassessment skills to develop and provide individual occupational therapy treatments and programmes for clients.
• Demonstrating compassion, empathy and sensitivity towards clients who may have complex emotional and physical conditions.
• Complying with local risk assessments when dealing with complex clients with challenging behavioural needs.

Detailed Job Description and Main Responsibilities

To be responsible for maintaining detailed and accurate patient treatment records in line with RCOT and legal standards. To be professionally and legally accountable for all aspects of own work ensuring that the standard of professional work meets professional standards and clinical guidelines.

Person Specification

Education/Qualification:

• Diploma or degree in Occupational Therapy
• Current HPC Registration
• Post Graduate qualification in relevant field or equivalent combination of degree, short courses and experience
• Evidence of CPD relevant to the treatment of people with physical disabilities and those with falls risks factors.
• PSI (Postural Stability Instructor) Accredited Training
• Member of Specialist Body e.g. COT SS-Older people
• Training in appraisal and clinical supervision
• BAOT Membership
