The Herts Unplanned Care Team is expanding. We are looking for proactive and enthusiastic Healthcare Assistants (HCAs) to join us and enable the delivery of care to patients in their own homes. The aim of the service is to avoid unnecessary admissions into secondary care and provide a timely discharge from hospital and community bedded units for patients requiring urgent therapy input in their homes. The team provides care across multiple pathways delivered over 7‑days a week from 8am to 10pm – Rapid Response (RR), Early Intervention Vehicle (EIV) and Discharge to Assess (DTA), General Medical Pathway (virtual Hospital). All pathways are delivered in a patient's usual home environment.

Responsibilities

  • Follow plans set by assessing clinicians and recognize changes to patients' conditions, reporting to the appropriate clinician as required.
  • Keep accurate clinical records and demonstrate good clerical/administrative skills.
  • Provide care across the 7‑day, 8am‑10pm service model.

Qualifications

  • Motivated team player with strong organisational skills.
  • Self‑motivated and capable of making decisions under supervision of senior team members.
  • Previous NHS experience is ideal but not essential; community experience desirable; training will be provided.
  • Good clerical/administrative skills and attention to detail.
