Join our Community Nursing service in West Hertfordshire. We are developing new community pathways to deliver more care in people's homes. This role is for a Band 6 Registered Nurse to lead a team of community nurses and support learning through the CLCH Academy.
Responsibilities:
- Set and maintain standards of care and deploy, coordinate and supervise the nursing team and learners as required.
- Provide leadership and management to community nurses and other staff across four localities in Herts Valleys.
- Be responsible for the team's assessment, planning, implementation and evaluation of individualised nursing care to patients living in the community (service provided seven days per week 8am-6pm).
- Maintain patient and other records.
- Participate in the long‑term management of patients within the locality and have continued responsibility for a defined caseload.
- Develop and maintain competencies within the team.
Qualifications & Experience:
- Band 6 Registered Nurse with experience in community nursing.
- Strong collaborative working style, approachable and effective communicator.
- Confident working independently and as part of a team, open to new ideas and ways of working.
- Full, valid UK driving licence and access to a car for business purposes (unless having a disability as defined by the Equality Act 2010).
