CLCH NHS Trust provides a wide range of community healthcare services, and the Hertfordshire Division is looking for experienced and motivated Bank Band 6 Unplanned Care Registered Nurses to support the delivery of high-quality, responsive care to adults in the community.

This is an exciting opportunity to join the Unplanned Care Service, supporting patients who require urgent assessment, treatment and ongoing clinical management within a community setting.

  • Undertake comprehensive nursing assessments of patients presenting with urgent or unplanned healthcare needs within the community.
  • Deliver safe, effective and evidence-based nursing care in accordance with Trust policies, procedures and professional standards.
  • Recognise and respond appropriately to changes in patients' clinical conditions, escalating concerns promptly to the appropriate clinician or emergency services.
  • Support the assessment, treatment and ongoing management of patients with acute, complex or deteriorating health needs.
  • Develop, implement and review individualised care plans in collaboration with patients, carers and the multidisciplinary team.
  • Administer medications and undertake clinical procedures within the limits of competence, authorisation and local policy.
