Central London Community Healthcare NHS Trust (CLCH) is looking to recruit a Team Lead - Therapist for an exciting opportunity within the intermediate care team (ICT). Are you a Band 7 or an experienced Band 6 Occupational Therapist or Physiotherapist looking for an exciting, new challenge and a chance to take on greater responsibilities? The ICT covers several community pathways involved in supporting hospital flow and supporting patients in the community in Wandsworth. These are:
- Care transfer hub - facilitating discharge and hospital flow in partnership with the St Georges discharge transfer of care hub.
- Discharge to assess community therapy team - follow up in the community of patients discharged from hospital to ensure smooth transition into community and avoid readmission.
- Quickstart service - provides rapid response social care services for patients who are at risk of social care breakdown in the community or to help facilitate hospital discharge.
- Phlebotomy - provision of phlebotomy for housebound patients.
The B7 will lead one of the primary pathways above, with the expectation to cross-cover across all pathways within the intermediate care team. Staff are expected to utilise CLCH bases across Wandsworth if necessary.
Main duties of the job
The ICT provides a 7 day a week service. Examples of duties are below:
- Liaising with the St Georges transfer of care hub, emergency department and ward teams to ensure timely discharge and linkage with CLCH service.
- Community therapy assessment and rehabilitation for patients recently discharged from hospital. These patients are usually more urgent / complex / have higher risk.
- Supporting the Quickstart care service with management of patients referred for urgent care.
- Risk management, disability management and rehabilitation are key components of the service delivery.
The team sees a wide variety of conditions/pathologies and provides therapy in a holistic way using a wide variety of knowledge and skills. The team consists of Physiotherapists, Occupational Therapists, rehabilitation assistants / healthcare assistants & phlebotomists in a multidisciplinary team.

Person Specification
Qualifications
- Diploma / degree in Physiotherapy or Occupational Therapy.
- Current AHP with UK HCPC registration.
- Evidence of continuing personal and professional development with up-to-date CPD portfolio.
- Membership in relevant professional body - Royal College of Occupational Therapists, Chartered Society of Physiotherapists.
- Clinical educator training / mentoring training.
- Leadership and management training.
Experience
- Appropriate post-qualifying >5 years' experience in a range of areas such as acute care of the elderly, neurology and community.
- Previous NHS experience in acute or community setting.
- Significant post-registration experience at band 6 level in a wide range of clinical areas including care of the elderly and patients with long term conditions.
- Experience of effectively working in collaboration within a multi-disciplinary team, negotiating and liaising and across organisational boundaries.
- Experience of collaborative working with other care providers e.g. acute, community, social care and voluntary services.
- Experience of leadership including coordinating and managing a team.
- Experience of supervising, teaching, appraising and mentoring juniors/assistants/students.
- Experience in presenting and service promotion.
- Experience in monitoring clinical standards.
- Experience of working in a changing and uncertain environment.
- Experience of acute/community discharge based or system flow operations.
- Experience in quality improvement or service improvement and implementing change.
- Experience of conducting research projects and audits.
- Experience of using an electronic clinical records system.
