Hospital at Home Practitioner in Sudbury

We are recruiting X2 full-time Band 6 Hospital at Home Practitioners to join our Brent Unplanned Care service, based at Sudbury and Wembley. This is an exciting opportunity to be part of an innovative community service delivering acute, hospital-level care directly in patients' homes. The Hospital at Home service provides short-term, intensive care for adults with acute conditions who would otherwise require admission to hospital. Working within a multidisciplinary team of specially trained registered professionals, including GPs, you will support earlier discharge, prevent avoidable hospital admissions, and ensure patients receive the right care, in the right place, at the right time.

You will have access to home-based monitoring equipment, point-of-care testing, and the ability to deliver a range of clinical interventions, including intravenous therapy, enabling you to practise autonomously while being fully supported by an experienced team. The post holder will work both autonomously and collaboratively with Acute Trusts, the Adult Community Nursing Team, and a range of health and care partners, including statutory and voluntary agencies, to deliver high-quality, person-centred patient care. The post holder will be required to:

• Work on a rotational shift basis within the Harrow Hospital at Home service, operating between 8am and 8pm, seven days a week.
• Effectively prioritise and manage their own workload, working with a high level of autonomy and minimal supervision.

Person Specification

Education/Qualification

• Essential: Registered Healthcare professional
• Appropriate Registration with relevant professional council
• Desirable: Advanced Clinical Assessment and Examination qualification
• Member of a Special Interest Group relevant to this post

Experience

• Essential: Demonstrate 2 years' experience post-qualification
• Previous experience of undertaking comprehensive health needs assessment and writing care plans
• Demonstrate knowledge of the purpose and value of supervision.
• Experience of teaching of junior staff / students, peers in-service training setting
• Experience of negotiating and communicating within a multi-disciplinary team
• Experience of teaching, patient and public involvement and health promotion in practice
• Experience of providing acute care response to frail and elderly patients in acute or community setting
• Experience of working in the NHS
• Desirable: Participation in research, service development or audit projects
• Previous experience working in community setting

Skills & Knowledge

• Essential: Evidence of effective team working
• Teaching relatives and carers across all areas of care
• Ability to demonstrate organisational skills and prioritisation skills.
• Ability to demonstrate and show evidence of effective clinical skills and delegation of clinical caseload to other members of staff within service

Key Attributes

• Essential: Skills in literacy, numeracy and computer skills (eg Microsoft Word, Power point, Excel), and efficient documentation skills.
• Computer literate with knowledge or experience of use of assessment templates and clinical systems such as EMIS.
• Hold a full, valid, UK driving licence and have access to a car to use for business purposes (unless you have a disability as defined by the Equality Act 2010)
