The Hospital at Home service will provide intensive hospital level care for acute conditions that normally require an acute hospital bed, in a patient's home for a short episode through multidisciplinary healthcare teams. The service will take appropriate patients out of hospital early to help release capacity in the hospital, deliver appropriate care and interventions and prevent unnecessary admission or attendance at hospital. The service is delivered by a team of specially trained registered professionals including GPs who visit each patient in their home to deliver the care they need.
Main duties of the job
The post holder will work autonomously and in collaboration with Acute Trusts, the Adult Community Nursing Team, other health care professionals and statutory and voluntary agencies to deliver excellent standards of patient care. The post holder will be required to:
- Work on a rotational shift basis within the Harrow H@H service operating hours 8 am - 8 pm, 7 days a week.
- Prioritise and manage own workload with minimal supervision.

Education/Qualification
- Registered Healthcare professional
- Appropriate Registration with relevant professional council
- Evidence of post-registration education and training
- Advanced Clinical Assessment and Examination qualification

Experience
- Demonstrate 2 years' experience post-qualification
- Previous experience of undertaking comprehensive health needs assessment and writing care plans
- Demonstrate knowledge of the purpose and value of supervision.
- Experience of teaching of junior staff / students, peers in-service training setting.
- Experience of negotiating and communicating within a multi-disciplinary team
- Experience of teaching, patient and public involvement and health promotion in practice
- Experience of providing acute care response to frail and elderly patients in acute or community setting
- Experience of working in the NHS
- Previous experience working in community setting

Key Attributes
- Skills in literacy, numeracy and computer skills (eg Microsoft Word, PowerPoint, Excel), and efficient documentation skills.
- Computer literate with knowledge or experience of use of assessment templates and clinical systems such as EMI
- Hold a full, valid, UK driving licence and have access to a car to use for business purposes (unless you have a disability as defined by the Equality Act 2010)
