Senior Rehabilitation Assistant

CLCH Hertfordshire Adult Community Services - CLCH NHS Trust provides a range of Community Services, and the Hertfordshire Division is recruiting! We are seeking an experienced Rehabilitation Assistant to join our specialist Community Neuro Service within South and West Hertfordshire. You will support people with a range of neurological conditions, including acquired brain injury and progressive neurological disorders, to maximise independence through rehabilitation and disability management.

Contributing to both physical and cognitive rehabilitation programmes, working under the guidance of experienced clinicians. The post involves working across a variety of settings, including patients' homes across West Hertfordshire, our newly refurbished community rehabilitation centre, and other community venues. There will also be opportunities to deliver virtual support and contribute to group-based therapy. You will be supported through structured supervision, with access to ongoing training and development via the CLCH Academy, and opportunities to contribute to service development within a forward-thinking neurorehabilitation team.

Due to the nature of this role, with travel between patient homes, there is a requirement to hold a full, valid, UK driving licence and have access to a car for business purposes (unless you have a disability as defined by the Equality Act 2010).

Main duties of the job

• Work as an active member of the multidisciplinary neuro rehabilitation team, delivering high-quality, evidence-based care to patients with neurological conditions, including Acquired Brain Injury, within the community.
• Accept referrals from qualified therapy staff and support the delivery of personalised rehabilitation programmes tailored to individual patient goals.
• Deliver therapeutic interventions both independently and under the guidance of registered therapists, ensuring care remains patient-centred and goal-focused.
• Manage and plan a designated caseload with support and supervision, prioritising workload effectively to meet service demands.
• Work as a lone practitioner in community settings, including patients' homes, implementing agreed treatment plans safely and effectively.
• Monitor patients' condition and progress, accurately recording outcomes and promptly reporting any changes or concerns to the wider therapy team.
• Contribute to the ongoing review and adjustment of rehabilitation plans in collaboration with qualified staff.

Person Specification

• Experience
• Recent experience of working in a relevant health or social care environment.
• Experience in rehabilitation.
• Knowledge of rehabilitation within neuro rehab.
• Experience working with people with physical, cognitive, behavioural and communication disabilities.
• Education/Qualification
• NVQ in Health Studies Level 3 or equivalent knowledge acquired through training, short courses or experience in health or wellbeing.
• General school qualifications i.e. GCE/GCSE Maths and English.
• Short courses in neurology and/or rehabilitation.
• Skills & Knowledge
• Good standard of written and spoken English.
• Ability to work independently and as part of a team.
• Understanding of the principles of rehab.
• Basic understanding of anatomy and physiology.
• Commitment to further learning & development.

£31,081 to £33,665 a year, inclusive of HCAS.
