Join our Brent Urgent Community Response (UCR) Team as an Occupational Therapist and make a meaningful difference to people's lives. You will deliver expert, home-based care that supports individuals to remain independent, recover safely and avoid unnecessary hospital admission.

As a valued member of our multidisciplinary team, you will provide expert occupational therapy assessments and interventions in people's homes, supporting recovery, promoting independence and enabling individuals to remain within their communities whenever possible.

Working closely with colleagues across health and social care, you will contribute to a dynamic service that responds quickly to changing needs and makes a real difference to people's lives every day.

Main duties of the job

The post holder will provide high-quality occupational therapy assessment, intervention and rehabilitation to patients across a range of community settings, including care homes, residential units and individuals' own homes.

You will work closely within a multidisciplinary team to support admission avoidance, facilitate early hospital discharge and promote independence and recovery.

Key responsibilities include:

• Undertaking holistic assessments and delivering evidence-based interventions tailored to individual needs.
• Prescribing, ordering and providing education on the safe use of equipment and adaptations.
• Contributing to effective care planning and risk management to support safe care at home.
• Supervising, mentoring and supporting the development of junior staff and students.
• Building strong, collaborative relationships with patients, families, carers, colleagues and partner agencies.
• Working flexibly and contributing to the delivery of a responsive, patient-centred service that meets the needs of the local population.

Person Specification

Education/Qualification

• Diploma/Degree in Occupational Therapy
• Registration with the Health and Care Professions Council

Experience

• Relevant work experience in a variety of clinical settings.
• Evidence of up-to-date CPD.
• Experience working as part of an MDT.
• Use of functional and cognitive assessments.
• Evidence of previous employment as an Occupational Therapist at Band 5 grade.
• Experience working in the care of elderly and with acutely unwell patients.
• Experience in environmental assessment and minor adaptations.
• Experience as a community equipment store Trusted Assessor.

Skills & Knowledge

• Knowledge of current best practices in Occupational Therapy and demonstrate clinical reasoning, applying theory to practice.
• Demonstrate an ability to work single-handedly with individuals or in a team.
• Understanding of professional ethics and their application in practice.
• Adherence to national, professional, and work-related policy.
• Health, safety, and risk awareness.
• Demonstrate an understanding of clinical governance and its implications for the profession.
• Basic knowledge of health legislation and current practice, risk assessment and management.
• Good organisational skills, ability to prioritise and manage time/caseload effectively.
• Effective written and oral communication skills. Ability to listen and present written and verbal information concisely and logically.
• Good IT skills (word processing and internet).

• Awareness of the legal responsibilities of the profession.

Salary

£36,943 to £44,900 a year inclusive of HCAS.
