An exciting opportunity for a compassionate and dynamic nurse to join a well-established nurse‑led service that delivers health care to people experiencing homelessness in North West London.

Role Details

Full‑time position, 8 months, supporting the Borough of Westminster.

The role focuses on post‑discharge care from hospital into accommodation settings, improving community health of those with complex needs, and preventing readmission.

Responsibilities

  • Support people experiencing homelessness on discharge from hospital into various accommodation settings.
  • Provide clinicians care to those with complex needs to improve health in the community and prevent hospital admissions.
  • Work closely with specialist hospital inclusion health teams, hostel staff, outreach services, and day‑centre staff.
  • Deliver person‑centred care, identifying unmet health needs and supporting individuals to meet those needs.
  • Use advanced nursing skills to conduct autonomous holistic assessment, diagnosis, and management of homeless patients with acute or undiagnosed conditions, injuries, complex health needs, and long‑term conditions.
  • Provide complete episodes of care within clinical guidelines, using expert judgement where necessary.
  • Deliver advanced nursing care in clinic settings and during outreach to street or other locations.
  • Partner with other community teams and voluntary organisations to ensure seamless delivery of care and proactive engagement with underserved groups.
  • Advocate for people experiencing homelessness, assist patients to overcome barriers, support their care choices, and promote independence and self‑care.
  • Manage a caseload and provide hands‑on care as appropriate.

Qualifications & Experience

  • Registered nurse with clinical skills suitable for providing hands‑on care.
  • Non‑judgemental, skilled, dynamic, with a commitment to high‑standard care for a vulnerable group.
  • Excellent problem‑solving, flexible, able to work autonomously and as part of a team.
  • Excellent communication skills; experience working in the community is beneficial.
  • Ability to work Monday to Friday.
  • Experience in advanced nursing practice preferred (not mandatory).
