We are looking to recruit a motivated Band 6 Physiotherapist to join the Urgent Community Response (Rapid Response) Team within the Harrow Integrated Care Team. You will work autonomously in the community, providing rapid assessment and intervention for patients referred from primary care, the London Ambulance Service, Accident and Emergency departments and other community services. The service aims to respond within two hours of referral, delivering timely, high quality care to prevent unnecessary hospital admissions.

This role offers opportunities to develop both clinical and leadership skills, with access to additional training such as venepuncture and catheterisation. You will work closely with multidisciplinary colleagues to support safe patient care, including follow up visits, escalation of deteriorating patients, and supporting safe discharge from hospital.

Main duties of the job:
  • Deliver high quality, evidence based care within the Urgent Community Response service, respecting patients' preferences, dignity, and cultural or religious needs.
  • Work autonomously to assess, treat and manage patients in the community, using clinical judgement to support safe decision making.
  • Contribute to the continuous development of the service through quality improvement and best practice.
  • Identify and respond appropriately to patients who are deteriorating, escalating concerns in line with service pathways.
  • Develop clinical and leadership skills, including undertaking additional training such as venepuncture and catheterisation.
  • Work flexibly across the service operating hours, supporting delivery of care seven days a week (Monday to Sunday from 8:00am to 22:00).
  • Hold a full, valid UK driving licence and have access to a vehicle for business use (reasonable adjustments will be considered in line with the Equality Act 2010).

Person Specification:

Qualifications:

  • Registered Allied Healthcare Professional with current registration with appropriate professional body.
  • BSc in Physiotherapy, higher or equivalent.
  • Qualification in teaching or practice-based learning.
  • Leadership course, e.g. Edward Jenner Leadership Course.
  • Physical Assessment Course or willingness to undertake.

Experience:

  • Substantial Band 5 experience.
  • Experience of assessing and providing quality care to people with Long Term Conditions.
  • Experience of supervising staff and completing appraisals.
  • Extensive range of clinical skills and experience.
  • Experience of working in a multidisciplinary team.
  • Substantial NHS Experience.
  • Experience of implementing evidence-based practice and supporting others to do so.
  • Experience of working in Rapid Response/ Primary Care/ Accident & Emergency.
  • Previous Band 6 experience.
  • Knowledge of clinical governance processes, assessing and managing risk.
  • Evidence of comprehensive range of clinical skills and expertise relevant to the role.
  • Basic IT skills including use of Microsoft Office package and email.
  • Clinical knowledge of presentation and management of conditions, illnesses and injuries affecting adults.
  • Knowledge of current NHS organisational structures and policy initiatives.
  • Knowledge of recent developments in relevant specialties.
  • Knowledge of management of adult and children safeguarding concerns.
  • Knowledge of appropriate Code of Professional Conduct.
  • Ability to complete and act on Annual Personal Development Reviews.
  • Detailed knowledge of collaborative working with other care providers, e.g. social services and voluntary services.
  • Knowledge of evidence-based practice and ability to implement into practice.
  • Knowledge of clinical supervision.
  • Awareness of equal opportunities in practice.
  • Willingness to obtain skills such as venepuncture and catheterisation/catheter care.
  • Experience with students.
  • Working knowledge of Clinical governance and evidence based practice.
  • Knowledge of the process of service evaluation and quality improvement methods.
  • Experience of providing / arranging in-service training.
  • Knowledge and experience in completing audits.

  • Must hold a full, valid, UK driving licence and have access to a car to use for business purposes (unless you have a disability as defined by the Equality Act 2010).

£44,485 to £52,521 a year per annum inclusive of HCAS.
