The Harrow Urgent Community Response service aims to prevent unnecessary hospital admissions by providing urgent and high-quality care in a community setting. Patients are referred to the team from primary care, London Ambulance Service, local Accident and Emergency departments, and other community services.
Responsibilities
- Within 2 hours from referral, obtain a detailed history from patients, families or carers, and complete a comprehensive physical assessment.
- Use assessment findings, diagnostic tools such as blood results, and additional scans (e.g., Duplex for DVT, x-rays) to formulate a diagnosis and provide treatment.
- Monitor patients through follow-up home visits, identifying deteriorating patients and acting on such findings.
- Support the Accident & Emergency department in the process of safe discharge and preventing admissions.
- Work autonomously, contribute to quality improvement initiatives, and expand clinical and leadership skills.
- Receive training in venepuncture and catheterisation.
Work Schedule
The service operates Monday to Sunday from 8:00am to 10:00pm. Full-time staff are allocated 13 long shifts and one short shift across a 4-week rota period.
Qualifications & Requirements
The candidate must hold a full, valid UK driving licence and have access to a car to use for business purposes (unless you have a disability as defined by the Equality Act 2010).
