The NHS Central London Community Healthcare (CLCH) Trust is seeking a motivated and enthusiastic pharmacist to join the Community Diabetes Team. This specialist clinical role delivers high-quality, patient-centred diabetes care within community settings. The post holder will provide expert pharmaceutical input, focusing on medicines optimisation, safety, and personalised care for people with complex diabetes needs. The role aims to improve glycaemic control, reduce health inequalities, and prevent complications.
Working within a multidisciplinary diabetes team, the Community Diabetes Pharmacist will collaborate with nurses, dietitians, consultants, GPs, and other health professionals to deliver integrated care closer to home. The role also contributes to clinical service delivery, patient education, quality improvement, service development, and workforce training, supporting CLCH's commitment to high standards of governance and population health management. Main duties of the job
- Provide expert pharmaceutical leadership within the Community Diabetes Service, ensuring safe, effective, and evidence-based use of medicines while promoting high standards of clinical practice and patient care.
- Lead and collaborate in multidisciplinary care, working closely with diabetes specialist nurses, dietitians, consultants, GPs, and other health professionals to deliver integrated, patient-centred community care.
- Drive medicines optimisation and clinical quality, supporting improvements in patient outcomes, safety, adherence, and alignment with national and local best practice guidelines and pathways.
- Undertake structured medication reviews, including complex patients with comorbidities, polypharmacy, and high-risk treatments, and support initiation, titration, and monitoring of diabetes therapies, including injectable treatments, within scope of practice.
- Support education, training, and workforce development, acting as a clinical resource and mentor for colleagues, students, and learners, promoting continuous professional development and a culture of learning.
- Contribute to service development and strategic initiatives, including the design, implementation, and evaluation of clinical pathways, quality improvement projects, and population health management programs to improve community diabetes care.
- Ensure compliance with governance and professional standards, including clinical audit, risk management, quality assurance, and adherence to Trust policies, regulatory requirements, and professional standards.
- Maintain professional development, innovation, and best practice, ensuring the role reflects current evidence, prescribing standards, and evolving models of care in community diabetes services.
- Provide education and counselling to patients and carers, supporting self-management, understanding of medicines, and lifestyle optimisation.
- Identify and manage medicines-related risks, including adverse drug reactions, drug interactions, and high-risk medicines, escalating concerns appropriately.
- Maintain accurate clinical documentation in line with Trust and information governance standards, supporting continuity of care and audit requirements.

Person Specification
- Registered Pharmacist with the General Pharmaceutical Council
- Masters Degree in Pharmacy or equivalent
- Higher degree / Postgraduate Diploma in Clinical Pharmacy or equivalent postgraduate diploma in specialist area
- Registered Independent Prescriber
- GCSE grade A-C or equivalent in English and Maths
- ECDL certificate
- Registration with the national regulatory body (when it becomes available)
Experience
- Post-registration experience as a Responsible Pharmacist in a busy community or hospital pharmacy.
- NHS prescribing awareness (formularies, etc.) which may have been achieved through dispensing hospital outpatient scripts or review of discharge prescriptions in community.
- Experience with the use of Microsoft Office and a variety of databases.
- Community pharmacy experience.
- Experience with the NHS patient information database (System 1).
