Responsibilities include setting and maintaining standards of care, deploying, coordinating, and supervising the nursing team and learners as required. Manage a caseload of patients within residential care settings, providing clinical leadership and supporting day-to-day management of the team. Line-manage and support Band 5, 4, and 3 staff, helping to develop confident, autonomous practitioners whilst ensuring delivery of safe, effective, and compassionate care.

Provide leadership and management to a team of community nurses and other staff covering the four localities across Hertfordshire. Be responsible for the team’s assessment, planning, implementation, and evaluation of individualised nursing care to patients living in a residential home. The service operates seven days per week from 8pm to 8am. Maintain patient and other records for the team. Participate in the long-term management of patients within the locality. Have continued responsibility for a defined caseload.

This role is ideal for an experienced community nurse looking to develop their leadership skills within a supportive and forward-thinking team. The post holder will be required to work across the locality in response to service needs, including participation in a 1 in 4 weekend rota. As travel between care homes and community settings is required, a full UK driving licence and access to a vehicle for work purposes are essential.
