Look no further - Barnet Learning Disability Service have an exciting opportunity for a Band 6 Learning Disability Nurse to join their dedicated and forward-thinking team. The post-holder will work as part of a multi-disciplinary team to provide specialist Nursing support to individuals with physical health needs accessing GP services. Main duties of the job
- You will be part of a friendly and dynamic Learning Disability Nursing Team and can expect to utilise your knowledge, skills and experience by promoting excellent quality care through evidence-based practice and value-based approaches to People with a Learning Disability within the London Borough of Barnet.
- Work collaboratively within a multi-disciplinary team to deliver safe, effective and person-centred care for individuals with learning disabilities across community and primary care services.
- Carry out holistic nursing assessments and develop, implement and review care plans, ensuring interventions are evidence-based and tailored to individual needs.
- Monitor and evaluate treatment plans, including medication regimes, taking appropriate action and escalating concerns to ensure high standards of clinical care.
- Act as a Care Coordinator for a defined caseload, including individuals with complex physical health needs, demonstrating strong organisational and prioritisation skills.
- Promote equality of access to healthcare by identifying and addressing barriers experienced by people with learning disabilities, supporting improved health outcomes.
- Work in partnership with service users, families, carers and professionals, providing specialist advice and guidance to support effective care delivery.
- Contribute to continuous service improvement through participation in clinical governance, audit, research and pathway development.

Job responsibilities
- To work as part of the multi-professional team, together with other community and primary care services, providing a co-ordinated and integrated care package to people with learning disabilities.
- To have continuing responsibility for the nursing assessment, formulation and development of clinical interventions and the monitoring and evaluation of care plans.
- To assess and monitor the progress of any agreed treatment plans, supervise prescribed medication regimes, be aware of and reporting back any adverse reactions to the prescribing doctor.
- To advise, facilitate and educate clients, carers and/or families on the appropriate approaches that may be required in delivering the agreed treatment plan/care.

- Qualified Learning Disability Nurse or Adult Nurse with an interest and experience of working with people with a learning disability
- Dual qualification – ie Learning Disability and Adult Nursing/mental health
- Post qualification training relevant to Physical Health care and people with a Learning Disability

- Experience of working with people with a learning disability and complex physical health needs
- Post qualification experience in learning disabilities nursing in a community environment
- Experience of co-ordinating care for people on CPA
- Experience of working with a multi-disciplinary team
- Experience of working with social care
- Experience of working with people with a learning disability and challenging behaviour
