Central London Community Healthcare is recruiting a Heart Failure Specialist Nurse to join our Community Heart Failure service on a fixed-term contract or secondment until May 2027. You will manage a caseload in the community, delivering evidence-based care and education to help patients self-manage.
You will work across patients' homes and clinic settings, coordinating with primary and secondary care teams and contributing to heart failure care pathways in line with NICE guidance.
