CLCH Hertfordshire Adult Community Services – CLCH NHS Trust Band 6 role, with preference for applications from experienced Band 5 clinicians with a strong background in respiratory care who are looking to develop their skills within a supportive service.
Key Responsibilities
- Deliver specialist respiratory care across West Hertfordshire, including Hospital at Home (admission avoidance and early supported discharge, including Virtual Hospital), home oxygen assessment and review, and community clinics and home visits for patients with OSA, COPD, Asthma, Bronchiectasis and ILD.
- Work autonomously while being well supported by the wider multidisciplinary team.
- Compile comprehensive respiratory assessments for patients with complex needs and co‑morbidities, and develop evidence‑based individualised management plans.
- Implement specialist care programmes such as home oxygen therapy, pulmonary rehabilitation, medication optimisation, and psychological support interventions in line with national and local guidelines.
- Provide compassionate emotional support to patients, their families or carers living with long‑term and life‑limiting respiratory conditions.
- Deliver care through community home visits and nurse‑ or physiotherapist‑led clinics across West Hertfordshire.
Qualifications and Requirements
- Full, valid UK driving licence and access to a car for business use (unless exempted by the Equality Act 2010).
- Clinical experience and competence in managing respiratory conditions such as COPD, Asthma, Bronchiectasis, OSA and ILD.
- Experience or strong knowledge of home oxygen therapy and pulmonary rehabilitation.
- Strong communication and interpersonal skills, with a patient‑centred approach to care.
