BANK - Specialist Physiotherapist

Are you a Band 6 or an experienced Band 5 Physiotherapist looking for an exciting, new challenge and a chance to take on greater responsibilities? We are looking for a highly specialist, dynamic and motivated Physiotherapist with excellent clinical skills and a positive attitude and commitment to providing excellent holistic community-based care. The person will join our group of AHP staff in a permanent role and supporting an MDT community therapy service.

Discharge to assess (D2A) is a 7 day a week community assessment and rehabilitation team in Wandsworth. It provides therapy services to specific types of patients upon discharge from hospital. These patients are often more urgent and/or complex upon discharge to the community. The team supports the discharge to ensure readmission is avoided. This involves risk management, disability management and rehabilitation. The team sees a wide variety of conditions/pathologies and provides therapy in a holistic way using a wide variety of knowledge and skills. The team consists of Occupational Therapists in a multidisciplinary team.

Main duties of the job

• You will be expected to work as an integral part of a D2A team.
• As part of the wider Occupational Therapy team, you will deliver high quality, evidence based occupational therapy intervention.
• The role will provide leadership in the development and ongoing quality assurance of the service, undertaking evaluation, audit and contributing to research.
• You will be key in being part of service development and support the team to strive for clinical excellence.

Key roles of the post:

• Experience of working within a community team.
• Work as a highly skilled senior practitioner with expert knowledge and work alongside clinical lead to develop the service.
• Providing highly specialist occupational therapy care to a community based caseload.
• Carrying out robust triage of patients referred to the team to ensure their safety and optimal pathway allocation.
• Ability to provide leadership within the occupational therapy and wider team.
• Experience of line management and supervising others.
• Supporting and deputising for the Clinical Lead and Operational manager of the service.
• Opportunities to take the lead on service improvement opportunities, develop your managerial skills and suggest and lead on audit and research projects.

Person Specification

• Physiotherapist with appropriate Diploma/ degree.
• Current registration with HCPC.
• Evidence of relevant continuing professional education and training.
• Willingness to undertake further training as appropriate.
• GCSE grade A-C or equivalent in English and Maths.
• Registration with the national regulatory body (when it becomes available).

Experience

• Relevant post registration experience covering core rotations including community experience.
• Appreciation of constraints to working in the community.
• Ability to supervise juniors/assistant/students.
• Ability to delegate tasks.
• Ability to use electronic clinical system e.g. RIO, EMIS, Cerner.
• Experience of MDT working.
• Participated in clinical audit.
• Experience in research / literature reviews.
• Clinical Teaching.
• Previous NHS experience.
• Experience working in a community setting.
• Experience of providing supervision and delivering training.
• Experience of working within care home settings.
• Experience of service development and audit.

Skills

• Knowledge of PT assessment tools relevant to an adult client group.
• Knowledge of a range of relevant PT interventions relevant to the client group e.g. dementia, Parkinson's, COPD, Falls, etc.
• An understanding of the range and applications of adaptive aids.
• Knowledge of professional standards including record keeping.
• Knowledge of national policies and procedures relevant to adults.
• An understanding of the principles of clinical governance.
• Excellent communication skills and ability to build effective relationships.
• Self-management skills - managing time and workload.
• Excellent oral and written communication and presentation skills.
• Ability to work under pressure, prioritise effectively and manage competing demands.
