The Hospital at Home service will provide intensive hospital level care for acute conditions that normally require an acute hospital bed, in a patient's home for a short episode through multidisciplinary healthcare teams. The service will take appropriate patients out of hospital early to help release capacity in the hospital, deliver appropriate care and interventions and prevent unnecessary admission or attendance at hospital. The service is delivered by a team of specially trained registered professionals including GPs who visit each patient in their home to deliver the care they need. The post holder will work autonomously and in collaboration with Acute Trusts, the Adult Community Nursing Team, other health care professionals and statutory and voluntary agencies to deliver excellent standards of patient care.
Responsibilities
- Work on a rotational shift basis within the Harrow H@H service operating hours 8 am – 8 pm, 7 days a week.
- Be the Team Leader for designated shifts on a rotational basis and responsible for day-to-day service management.
- Take designated responsibility for discrete areas of operational management/clinical practice.
- Prioritise and manage own workload with minimal supervision.
- Be responsible for undertaking comprehensive health assessments, physical examination and ordering tests for patients in Urgent Community Response services.
- Use advanced clinical decision making and assessment skills, coordinate care planning and initiate referrals to diagnostics for appropriate tests for this population.
- Interpret complex information and findings from diagnostic tests and examinations and formulate solutions to inform on the best treatment plans for patients and carers.
- Be involved in establishing, delivering and leading a comprehensive service within community services.
- Fundamentally working as an autonomous and advanced practitioner assessing, diagnosing and treating patients in an appropriate and timely fashion.
- Demonstrate sound critical decision-making in the clinical setting.
- Work in line with all the relevant professional standards, codes of conduct, and organisational guidance.
