Job Family: Supporting - CBG7
Salary: £28,598 - £31,022 per annum pro rata
Hours: Full-time - 37 hours per week
Location: A combination of working within the community (across the county), local hospitals, from home and any CBC offices, including Priory House, Shefford. You will be expected to attend a CBC office location as often as needed to carry out your role, which will be no less than once per month.
Contract: Hybrid – Fixed Term Contract till 31st of December 2026.

About the role: Our Reablement & Step Up/Step Down services put residents at the heart of everything we do. Following an accident or illness, sometimes people need a helping hand to get back to everyday life. That’s where our Reablement Service comes in. Our goal is to help people to get back on their feet – and regain their independence.
Become a Reablement Co-ordinator, and you’ll find your greatness supporting our team of Reablement Workers & Response Workers as they care for those who need it most. You’ll enable them to do their job at the highest standards, arranging training, monitoring their performance, and ensuring procedures are followed, championing best practice.
You’ll carry out assessments independently or jointly with a therapist to determine the level of care and support people need – and put the wheels in motion to make it all happen. You’ll demonstrate excellent interpersonal skills and should be a natural carer.
For this role, it is preferable that you hold an NVQ Level 3 in Care (or should be able to demonstrate that you have worked within a senior care role), plus have a knack for writing care plans, coordinating people and adhering to CQC guidelines.
