Job Overview

The post holder will be responsible for delivering a robust integrated discharge model that ensures effective, person-centred and integrated pathways of care out of hospital are embedded through interdisciplinary ways of working and clear, well managed processes and protocols.

The Integrated Discharge Service is a collaborative and partnership approach with the Royal Free Hospital to coordinating and planning the transfer of patients, including those with complex health and social care needs, back to their usual place of residence wherever possible following an episode in hospital.

The post holder will also work with University College London Hospital to support the discharge of Camden and Barnet patients. The ‘Hospital Discharge and community Support policy Requirements’ document introduced during the first COVID wave in April 2020 and updated in October 2021 underpins the role including scope and expectations. It is acknowledged that as changes occur nationally, the scope of the role may change to meet requirements.

In addition to the Transfer of Care Hub role, the post holder will also be the lead for the Camden Community Access team, which is line managed by the Operational Support Manager. This team coordinates community health referrals within the borough of Camden.

Main duties of the job

The post holder will provide visible leadership for the whole Camden Community Access and Integrated Discharge Service and directly manage the team’s strategic and operational responsibilities. The post holder will receive and manage operational and clinical escalations and ensure a Homefirst approach is promoted. They will require:

  • Experience of working with senior leads in a range of partnership organisations
  • The development and maintenance of positive and productive relationships across current and changing team and organisational boundaries, often in pressurised situations
  • The delivery to timescale of key deliverables and performance targets understanding of the context within which the key health and care organisations operate and how to effectively align their priorities, statutory requirements and local pressures.

Qualifications & Training

Essential criteria

  • University degree or equivalent level professional qualification or experience
  • Administrative proficiency for accurate data management and reporting
  • Demonstrate capability and capacity for service management at a senior level in a complex healthcare environment, including staff management and change management
  • Relevant Professional Clinical, Social Work, AHP, Nursing qualification.

Desirable criteria

  • MSc/MA relevant to post (or equivalent) – achieved or working towards

Knowledge & Experience

Essential criteria

  • Previous managerial experience in a large and complex team/service/organisation
  • Previous experience working in a management role within Adult Services or physical healthcare services including comprehensive understanding of the discharge to assess principles.
  • Previous clinical experience of discharge within health and/or community settings
  • Demonstrate success in delivering change and performance with and through management of teams
  • Ability to work within scope of own role, delegating to others as appropriate to ensure best use of own skills and time
  • Ability to analyse complex problems and to develop practical and workable solutions to address them
  • Ability to think and plan strategically, tactically and creatively, and to prioritise work programmes in the face of competing demands
  • Ability to manage and deliver to deadlines and within resources. A strong sense of personal and team accountability coupled with a clear understanding of the boundaries around delegated authority
  • Ability to collaborate constructively with internal and external partners to create the conditions for successful partnership working
  • A good understanding of the changing NHS environment; particularly in relation to community services, primary care networks and integrated care systems
  • A detailed understanding of the national agenda for health services – both legislative and direction of travel
  • Track record of undertaking and completing significant projects
  • Ability to relate to people of all disciplines and at all levels within an organisation
  • Prioritising and managing workload within a busy operational role
  • Self-motivated and autonomous

Desirable criteria

  • Breadth of management experience in different environments/across different areas (NHS or non-NHS)
  • Knowledge of the NCL discharge pathways and processes

  • Advance keyboard skills knowledge of a full range of IT software including databases, Microsoft Office and MS Teams

  • Ability to be intellectually flexible and to look beyond existing structures, ways of working, boundaries and organisations to produce more effective and innovate service delivery and partnerships

  • High level of work organisation, self–motivation, drive for performance and improvement, and flexibility in approach and attitude
