Main duties of the job
The purpose of this post is to work as a member of the multi-disciplinary clinical team within a prison healthcare setting. Your role would involve working compassionately with young adults with neurodevelopmental & mental health needs within the prison, either within the inpatient unit and/or on the main wings/houseblocks. You will focus on the holistic needs of service users, working across boundaries of care, organisations and roles coordinated through the Care Programme Approach within a trauma informed care framework.

Detailed Job Description And Main Responsibilities
Clinical:
- To work together with the service user and where possible their carers in the drawing up of care plans encouraging and motivating service users to take an active role in their own care plan.
- To contribute to the assessment, planning, implementation and review of care with the multi-disciplinary team
- Help to identify early signs of relapse by monitoring the service user’s progress, level of functioning and mental state and alert the appropriate staff involved in the service user’s care.
- To assist in the delivery of the therapy programme within the service by co-facilitating groups
- To liaise with and refer service users to other healthcare services to ensure continuation of appropriate treatment
- To be actively involved in regular multi-disciplinary risk assessments along with daily informal risk assessment when carrying out interventions
- Report regularly to qualified staff
- To maintain accurate, contemporaneous patient records as required by existing procedures, entering appropriate details on the service users case notes as necessary
- Participate in the review of the service provided to clients overall and advise any changes to the supervisor
- To undertake such other duties as may be determined from time to time within the general scope of the post
- To accept and respect service users personal beliefs, uniqueness and identity.

- To attending and actively participating in training sessions and team/Proximal network meetings.
- Receive and utilise clinical supervision and appraisal to develop and maintain safe practice
- To dress and behave in a professional manner appropriate to the role

- To fully participate in the Trust’s performance review and personal development planning process on an annual basis.

- Undertake training as necessary in line with the development of the post and as agreed with line manager as part of the personal development planning process.
- To participate in the service line learning & development meetings and training
Service Development and Delivery

- To maintain up to date knowledge of legislation, national and local policies and issues in relation to both the specific service user group, peer support and mental health.
- To comply with all relevant Trust policies, procedures and guidelines, including those relating to Equal Opportunities, Health and Safety and Confidentiality of Information and to be aware of any changes in these.
- To be aware of, teach others and challenge issues in relation to stigma, low expectations and anti-discriminatory practice as appropriate.
- To read and understand the statutory requirements of the Mental Health Act 1983.
- To attend clinical, business & service development meetings as required.
- To raise the profile of peer support both within CNWL, and externally, through co-delivery of presentations, workshops, and participation in Trust –wide peer related projects and audits: and information sharing as required and as agreed with the line manager.

- Ability & desire to obtain both underpinning knowledge and NVQ qualifications
- NVQ Level 4 or demonstrable equivalent in health or social care
- Commitment to undertake Continuing Personal Development

- Willing to undertake NVQ Level 4 or demonstrable equivalent in health or social care (if appointed through preceptorship)

Previous Experience

- Experience of mental health services (as a worker, service user or carer)
- Working within a Multidisciplinary Team (MDT)
