The Nurse Practitioner provides health promotion and maintenance through the diagnosis and treatment of acute illness and chronic conditions. Responsibilities include:
- Providing comprehensive care to seniors following clinical standards.
- Collaborating daily with your care team to ensure coordinated, high-quality care.
- Managing referrals, hospital/SNF coordination, DME, and home health services.
- Participating in daily huddles.
- Documenting care efficiently with support from quality-based coders.
- Participating in on-call rotation and supporting growth plans at your center.
- Ensuring compliance with all licensing and accreditation requirements.
Required Qualifications:
- Master's Degree in Nursing with board certification.
- Active, unrestricted NP license in the state of practice.
- Experience with value-based care and integrated team-based models.
Preferred Qualifications:
- Experience in value-based care, primary care or Medicare Advantage settings.
- Active DEA license and Medicare/Medicaid provider numbers or ability to obtain DEA license and/or Medicaid and Medicare numbers.
- Familiarity with HEDIS quality indicators and Medicare guidelines.
