Role Overview: The Nurse Practitioner provides health promotion and maintenance through the diagnosis and treatment of acute illness and chronic conditions. Prescribe medication, examine patients, diagnose illnesses, and provide treatment.
Responsibilities:
- Provide comprehensive care to seniors following clinical standards.
- Collaborate daily with your care team to ensure coordinated, high-quality care.
- Manage referrals, hospital/SNF coordination, DME, and home health services.
- Participate in daily huddles.
- Document care efficiently with support from quality-based coders.
- Participate in onâcall rotation and support growth plans at your center.
- Ensure compliance with all licensing and accreditation requirements.
Required Qualifications:
- Master's Degree in Nursing with board certification.
- Active, unrestricted NP license in the state of practice.
- Experience with valueâbased care and integrated teamâbased models.
Preferred Qualifications:
- Experience in valueâbased care, primary care or Medicare Advantage settings.
- Active DEA license and Medicare/Medicaid provider numbers or ability to obtain DEA license and/or Medicaid and Medicare numbers.
- Familiarity with HEDIS quality indicators and Medicare guidelines.

Work Hours & Pay:
Scheduled weekly hours: 40 hours per week. The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc. $115,200 - $158,400 per year. This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
