Center for Life Management (CLM) provides the area’s most comprehensive array of outpatient mental health services for adults and elders. Our Integrated Care Program is a highly innovative approach to whole healthcare that encompasses the integration of treatment modalities for mental and physical health needs. The Integrated Care (IC) team utilizes a wrap-around care model that is necessary to support individuals with addressing the intersection of their complex mental and chronic physical health conditions.
The Program Support Specialist provides administrative, operational, and programmatic support to ensure the smooth execution of organizational programs and projects within the Integrated Care department. In addition, the Program Support Specialist provides coordination of care for clients in the Healthy Together primary care program. This is a part time, 25 hour a week position. Schedule: Monday – Friday 12 pm - 5 pm.
Major Functional Areas and Description of Duties
- Provide comprehensive administrative and operational support to include:
- Greet and assist clients and visitors professionally during check-in and check-out procedures.
- Accurately collect co-payments, outstanding balances, authorizations and required signatures; ensure all payments are properly recorded.
- Scan and correctly label documents into the electronic medical record system, maintaining accurate and organized records.
- Support clinical staff by conducting reminder calls for appointments, booking follow-up appointments, and conducting outreach for missed appointments.
- Monitor and restock office supplies, equipment, and other materials to be readily available for staff use.
- Promptly relay concerns, questions, and/or important information to the appropriate manager, clinical supervisor, administrative team member, clinical and/or medical staff.
- Follow opening/closing protocol ensuring safety of staff, clients, and facility.
- Prepare assessment packets and necessary documentation before clinical assessment appointments.
- Ensure timely and accurate documentation to support continuity of client care.
Monitor and support the established workplans of Integrated Care’s partnership with the NH Healthy Lives Program to include:
- Track referrals to and participation in internal and external evidence-based chronic disease groups by clients.
- Utilize monthly data reports provided by Lamprey Health Care to accurately and efficiently update health outcomes and monitor client chronic disease tracking.
- Collect, track, and maintain data related to physical activities surveys and enrollment in programming.
- Complete quarterly workplan updates and semi-annual Performance Measurement reporting and submit and review data with Director of Integrated Care.
- Work collaboratively with Transitions of Care Coordinator to research and enter client data into the Integrated Care patient registry to maintain accuracy.
Provide program support and client care coordination for Healthy Together Primary Care partnership to include:
- Facilitate weekly Healthy Together huddle and take detailed notes to include in documentation in EHR.
- Complete documentation of client updates in the EHR following huddle and send to treatment teams involved as appropriate.
- Contribute to program strategy development and provide feedback to improve program effectiveness.
- Ensure Healthy Together staff involved in a client’s care are up to date with case information for continuity of care.
- Evaluate and process new patient referrals for Healthy Together to ensure the accuracy of information and attachment of necessary documentation prior to forwarding to Lamprey Patient Services Representative for scheduling.
Perform functions professionally and in a manner that protects the integrity and confidentiality of clients and staff. Actively participate in appropriate team meetings and committees as requested by supervisor. Represent CLM in a professional and appropriate manner in all settings. Perform all other duties as assigned by supervisor.

Requirements Qualifications and Skills
- High school diploma or equivalent required.
- Minimum of two years in a medical office administrative role preferred.
- Strong communication, interpersonal, and customer service skills required to interact with clients, families, staff and visitors in a professional manner.
- Ability to prioritize tasks, manage multiple responsibilities, and meet deadlines in a fast-paced environment.
- Ability to maintain composure and exercise sound judgment in routine, urgent, and emergent administrative tasks.
- Proficient computer skills and knowledge of Microsoft office applications with emphasis on Word, Excel, Outlook and TEAMS.
- Ability and desire to work collaboratively as part of a team while also being self-motivated and capable of working independently.
- Flexibility to adapt to schedule changes and provide coverage as needed.
- Ability to learn and implement new skills as needed according to evolving department needs.
