CLM provides the area's most comprehensive array of mental health services for children and their families.

Our multi-disciplined treatment team works with individuals ages 3 – 21 who experience mental illness and/or serious emotional difficulties.

Assertive Community Treatment (ACT) is an evidenced based practice that provides an intensive and highly integrated approach to community mental health services.

ACT support helps at-risk children, youth and their families bring about positive change in symptoms and behaviors that impact school, relationships, living arrangements, and overall wellbeing.

ACT reduces mental health crisis, unnecessary hospitalization, encounters with law enforcement, involvement in the juvenile justice system, and out of home residential placement.

The

Children’s ACT Clinician to provide outpatient clinical services including diagnostic assessment, treatment planning, crisis management, individual therapy, family therapy and community-based support.

This is a full time, 40 hour per week position.

  • Major Functional Areas and Description of Duties
  • Provide individual diagnostic assessment to determine client’s strengths, needs, resources, and objectives.
  • Formulate treatment plans specifying presenting problems, objectives, interventions, and desired outcomes.
  • Complete assessments for eligibility determination for State funded services.
  • Provide therapy in the home and community, engaging natural supports as often as possible.
  • Provide crisis management and intervention when clients experience acute exacerbation of symptoms to ensure their safety within the home and/or community.
  • Provide timely documentation on each service delivered.
  • Work collaboratively with other ACT Team members, natural supports, and external providers (school, primary and specialty care physicians, legal system, etc.) to ensure wrap-around services are in alignment with the ACT model to meet client needs.
  • Work to foster individual recovery, resilience, and life skills to maintain independent living in the community.
  • Provide set hours of billable service each week to meet 55% key performance indicator expectation (eligible for incentive pay upon exceeding productivity expectation).
  • Serve on appropriate committees, both internal and external, as determined by supervisor.
  • Function professionally and in a manner that protects the integrity, confidentiality and rights of all patients.
  • Executes all required services and documentation in accordance with agency policies and federal/state standards.
  • Maintain professional memberships, licenses, accreditation, certifications, etc.
  • Represents CLM in a professional and appropriate manner in the office and community.
  • Performs all other duties as assigned by supervisor.
  • Physical Demands and Working Conditions

A majority of the work will be visiting clients in the community and also in their homes.

Manual dexterity is needed to use computers.

The ability to sit for extended periods of time is required.

Reliable transportation, a clean driving record, and proof of auto insurance coverage is required.

Requirements

Qualifications and Skills

  • Master’s degree required.
  • LISCW, LCMHC or LMFT preferred.
  • Previous experience providing family therapy and working within a multi-disciplinary team.
  • Ability to exercise sound clinical judgment in routine, urgent, and emergent clinical situations.
  • Ability to actively engage with children and conduct interactive play within a therapeutic setting.
  • Proficient computer skills, specifically with all Microsoft office applications.
  • Ability to document effectively and efficiently in course of clinical work.
  • Ability to navigate an electronic health record.
  • Ability and desire to work as part of a clinical team while also able to be self-directed and motivated with minimal supervision.
  • Ability to acquire and effectively practice new clinical skills as needed according to evolving agency needs/services.
  • Effective communication skills, organizational skills, and ability to multi-task.
