The role
Reporting to the Coordinator – Aged Care Services, the Clinical Care Partner plays a key role in supporting older Australians to remain safe and independent at home.
Based in Darra, this Full-Time role combines clinical assessment, health expertise and care coordination to deliver person-centered, evidence-based and culturally safe care.
You will work collaboratively with older Australians, families and carers, Care Partners, Support Workers, Allied Health Professionals, GPs and specialists to develop and deliver care that reflects each client’s individual needs, goals and preferences.
You will contribute to clinical governance, Restorative Care, End-of-Life pathways, multidisciplinary team (MDT) collaboration and support clinical-related investigations, including Serious Incident Response Scheme (SIRS) requirements.
Key Duties
- Complete clinical assessments, care planning, reviews and care coordination.
- Provide clinical support including wound care, medication management and dementia support.
- Monitor client health and wellbeing, escalating concerns and supporting timely interventions.
- Promote wellness, reablement and restorative approaches that maximise independence.
- Collaborate with clients, families, carers and multidisciplinary teams to achieve positive outcomes.
- Provide clinical guidance and support to Care Partners and Support Workers.
- Maintain accurate documentation and contribute to quality improvement and clinical governance activities.
- Support compliance with aged care standards, policies and regulatory requirements.
Skills and Experience
To be successful in this role, you will have
- A relevant tertiary qualification in Nursing, Physiotherapy, Social Work or another health discipline.
- Current professional registration where applicable.
- Experience in aged care, community-based clinical care, care planning and clinical decision-making.
- Strong knowledge of person-centred care, clinical governance and multidisciplinary practice.
- Excellent communication, relationship-building and cultural competency skills.
- Ability to work autonomously while contributing positively to a team environment.
Desirable Experience
- Experience supporting people living with dementia, palliative care needs or complex health conditions.
- Experience with Restorative Care, End-of-Life pathways or SIRS reporting.
- Experience mentoring or supporting other team members.
- Familiarity with My Aged Care systems and digital care management platforms.
Essential Requirements
- Current driver’s licence and reliable, registered and insured vehicle for work purposes.
- Current Blue Card.
- NDIS Worker Screening Clearance.
- Current First Aid and CPR certification.
- Australian Federal Police Criminal History Check.
- Evidence of right to work in Australia.
