Centacare Brisbane is seeking a Clinical Care Partner to deliver person-centred care, comprehensive clinical assessments, and ongoing care coordination to help older Australians live safely and independently in their own homes. The role involves full-time hours, Monday to Friday, primarily across Darra (South Brisbane) with travel to clients.
Salary is AUD 58 per hour plus superannuation, not-for-profit salary packaging and travel reimbursement.
